Welcome to the Peabody Public Schools!
We’d like
to assist you in registering your child in any one of our
schools by providing you with information related to
documentation you will need when you report to your child’s new
school. Parents or guardians enrolling children in Peabody
Public Schools for the first time must present the documentation
listed in the following checklist:
_____ Two (2) proofs of residence
This may be a driver’s license and automobile
registration and/or deposit receipts or current month bills
(electric, gas, cable, or telephone) indicating parent/guardian
and the new Peabody address.
_____ A certified birth certificate
_____ Current physical exam
This should include an updated immunizations record signed by a
licensed medical provider.
_____ Transfer certificates and/or school records from
previous school
_____ Individualized Education Plan (IEP), if
applicable
_____ 504 Accommodation Plan, if applicable
Also, a Student Registration Form must be completed for
each child. Clicking on the link below will provide you with
downloadable forms that you may print at home and complete prior
to your registration appointment. Doing so will speed up the
registration process.
If you are registering a child at Higgins Middle School (grades
6-8) or Peabody Veterans Memorial High School (grades 9-12),
call the school to schedule an appointment with a guidance
counselor.
If you are registering a child at one of the elementary schools
(grades K-5), call Peabody Public Schools’ attendance office at
