NOTICE from the SCHOOL NURSE for NEW STUDENTS

State Law mandates that prior to entry all new students must present the school nurse with up-to-date immunizations and a current physical examination.  Grades 6-12 require the following immunizations:

Dtap/DTP or 3 doses Td (plus 1 Td booster for all 7th or above)
Polio - 3 doses
MMR - 2 doses  (2 doses measles, 1 mumps, 1 rubella)
Heptatis B - 3 doses
Varicella - younger than 13 years, 1 dose; 13 years or older, 2 doses or a physician-documented history of the disease.

Medication Packets are available in the main office over the summer if your student requires medication during the school day.
